Application Instructions

Application Deadline: March 14, 2008

Students are required to submit a complete application package by the March 14 deadline. Incomplete packages will not be considered.

Application packages can be sent by regular mail IF they are mailed early enough to be received by the March 14 deadline.

NOTE: Students who choose to send the application package at the last minute are URGED to send it by a courier service that can guarantee delivery or postal service overnight delivery to ensure arrival by the March 14 deadline.

The package must contain the following items.

1. One printed and signed copy of the on-line application
2. One copy of high school transcript, official or unofficial
3. One letter of recommendation from a teacher, school administrator or someone who can attest to the student's academic qualifications
4. One signed Parental Consent and Rules of Conduct Form (download PDF)
5. One signed Talent Authorization Release Form (download PDF)
6. One signed Commitment Form (download PDF)

Students chosen to participate in VASTS will receive an email notification by March 27, 2008.

Online application

Once you have submitted the on-line application, you will receive an email confirming the submission. Please print this confirmation copy, sign it and submit it in your application packet.

Transcript

Your high school transcript can be official or unofficial (request this early from the school)

Letter of Recommendation

The letter of recommendation should be provided by a school teacher or administrator. If home schooled, the letter can be from someone who can attest to the applicant's academic qualifications. The person providing a recommendation can choose to complete it on-line, which is the preferred method, or by hard copy using the provided Letter of Recommendation Form. When filling out their application form the student will enter the name and email address of the person providing the recommendation. When the student submits their application, the on-line system will send an email to this person with specific instructions. Students should request the letter early to ensure that it is completed by the deadline. If the person supplying the recommendation chooses to provide a hard copy, it should be mailed with the application package.

Parental Consent and Rules of Conduct Form

The Parental Consent and Rules of Conduct Form must be downloaded, read and signed by both the parent/guardian and student and returned in the application package.

Talent Authorization Release Form

The Talent Authorization Release Form must be downloaded, read and signed by both parent/guardian and student and returned in the application package.

Commitment Form

The Commitment Form must be downloaded, read and signed by both parent/guardian and student and returned in the application package.
